Underage Athlete Forms

Important: These forms should be completed BEFORE creating an account for an underage athlete, as you will be required to include these forms when you create the account. When you complete the form, you will receive a PDF copy of your completed form via email. You will be prompted to upload the PDFs when creating the underage athlete’s account. For athletes with existing EI accounts, forms will be submitted directly to Head Office – no need to upload.
